A CHANCE TO THRIVE

A CHANCE TO THRIVE is a program focused on addressing your barriers to independence and equipping you with the skills to be successful and THRIVE in the community.

 

Participation in the program includes housing, meals, personal care products, and structured therapeutic services.

 

A CHANCE TO THRIVE will assist you in

  • Furthering your education

  • Job development

  • Job training

  • Permanent housing

  • Furnishing your apartment

Individual & Group Therapy 

Group and Individual sessions are designed to assist you to navigate life at your highest self.

By participating in group, you will gain insight into your own choices and behaviors from others who 'have been there'. As you form a community and learn from each other, you are better able to incorporate what you’re hearing from your support and care teams.

 

Intensive Outpatient Program

An outpatient treatment program used to address addiction, and mental health disorders. The goal is to help you manage your symptoms and develop healthy coping mechanisms as it relates to your recovery.

Case Management

Case Management services provides an array of services designed to connect you to community resources which include housing, employment, primary health care, etc.

These services are focused on helping you navigate daily living and thrive in the community.

Therapeutic Behavior Services

TBS services include crisis prevention, restoration of social skills and daily functioning, identify strategies of treatment options, interventions using evidence-based techniques, referral to other behavioral healthcare, and non-health care services.

Mental Health Day Treatment Services

Goal-oriented service that utilizes multiple mental health interventions to address your mental health needs. The purpose is to stabilize, increase or sustain your highest level of functioning. Interpersonal and social competency as age, developmentally, and clinically appropriate.

Shared Independent Living

A family like atmosphere created to provide women who wish to live independently but not alone, a safe and supportive environment to live in. Supportive mental health services, medication monitoring, personal care services (and items), meals, are all covered by the facility.

SUD Case Management

These services assist and support you in gaining access to needed medical, social, educational and other services essential to daily living.
